The detection of benzodiazepines in urine represents an analytical challenge that requires robust and standardized instrumental methods due to their structural diversity, the presence of conjugated metabolites, and the interferences inherent to the urine matrix. The objective of this systematic review was to analyze instrumental methods for the detection of this substance in urine samples, identifying their analytical performance and the factors influencing their selection. The methodology involved a systematic review of the literature following the PRISMA protocol and applying PICOS criteria to clearly structure the research questions. A total of 484 previous studies published between 2020 and 2025 from the scientific databases Scopus and Web of Science were analyzed, including scientific papers from journals in the Q1 and Q2 quartiles. Risk review tools AMSTAR-2, SANRA, AXIS, and ROBINS-I were applied. After the selection process, 43 primary studies were included, and their results show that the choice of method depends on available resources, personnel training, and the context of use, prioritizing speed in clinical settings and sensitivity and selectivity in forensic toxicology. In conclusion, LC–MS/MS is considered the ideal method for the accurate detection of benzodiazepines, while immunoassays are limited to their use in initial screening due to their limited sensitivity.
